My first thought was that the scanhead is unsure of its home position.

Apart from this, I can only assume that the belt is out-of-alignment and will need to be reseated, perhaps by following the service instructions found in the Xerox WorkCentre manual downloadable here. When youi have issues like this, the first place to check is the manufacturer's support website for current drivers and utility software. Installing driver from the manufacturer site doesn't solve this problem.
